Many countries are currently engaged in restructuring of their health care systems to take advantage of the efficiency and cost-savings afforded by EHRs. Unfortunately, there is a lack of global standards both for the relevant technology as well as with respect to user protocols and professional qualifications. This panel, to be held jointly by WG 1, WG 4 and JHIFT (Healthcare Information System Technologist Fostering Taskforce – Japan) will focus on the need for global standardization of certification requirements for health information professionals HIPs. Its purpose is to clarify some of the more troublesome issues in this regard and point the way towards a solution.

To inform about the impact of a recent movement towards a policy to develop integrative networked electronic health record (EHR) as a basis for cooperation among care teams and with patients and in support of safe patient care in Japan. The hospitals based that most have computer supported administrative procedures, but only few computer-based health records by means of the author performed surveys. The attitudes of the EHR users in Hiroshima area show that while they expect efficiency and quality improvements, there is also apprehension that the system in use might lower practical efficiency and compromise patient safety. Accordingly, health recording requirements and storage policy have been restructured and communicated to the hospitals.

As a basic medium of team medical care process under multi-specialties’ cooperation, EHR is required to secure patient safety and meet the information disclosure demand.  Based on the reassessment of the structure and description requirement of EHR, its next generation construction policy is proposed. Systematically, EHR is available for integrated description.  However, this result was caused by the subsystem construction for different users without sufficient understanding of medical care process. These insights led to the initiation of curricula educating “Health Information Technologist” which is promoted by Japan Association Medical Informatics. They will also contribute to improved and advanced EHR.

The Healthcare Navigation System (HNS) is designed as the core of the new information system of Hiroshima University Hospital. This system is expected to make the drastic change of the hospital information conception to provide the patient participatory health care. HNS is used not only by physicians, but also by all co-workers, which will enable them to improve team communication. Moreover, as the bedside information systems were set up at all of 700 beds, patients can access their anamnesis and future clinical care procedures.

Based on the demand above, we aim to make patients’ right compatible with attendants’ accountability.

The new information system in HU-MIND II—Hiroshima University Hospital Medical Intelligence and Notice Delivery system II is designed as “clinical management system” (CMS). The core of this CMS is the electronic health record (EHR), which aims to assure both the patients’ right to know, and the attendants’ accountability. It is evident that the team practice including patients requires close communication. Based on the demand outlined above, the new regulations are composed of 21 requirements, conditions of information collection, accumulation and use.

Background : On April 22, 1999, the Japanese Ministry of Health and Welfare announced their decision to permit electronic preservation of clinical records under the condition of keeping “authenticity”, “legibility”, ”preservation” and privacy protection”. This means that the rule does not require preservation of the paper documents for a certain period of time. The new general act for personal data protection has been prepared in Japan. It will come into force in 2001. The SRG of JAM has been concerned about its’ surplus regulation on “Private Data ”.  Aim : Above all, to improve healthcare service quality, we planned the junction module of the data retrieve and use structure in hospitals and amongst their outside facilities as the common media for the healthcare specialists.

On April 22, 1999, the Japanese Ministry of Health and Welfare proposed electronic preservation of all clinical records. Simultaneously, deliberations on legislation on data disclosure and privacy protection were held. To promote these movements, electronic use of Personal Health Data (PHD) is indispensable, and the system development is on its way to meet rising demands. Indigenous Japanese did not have a word to describe the concept of privacy. It was only in the 1960s when we became aware of that. In this article, the protection policy for data use will be discussed from a Japanese perspective compared with those in other nations, giving an example of the hospital management system under construction at the Hiroshima University Hospital.
